Chapter 1 of 9 · Start

For adults with ADHD

# The Rejection Alarm

A late text. A small frown. "This needs work." For some people it lands like a punch.
Nine short chapters explain why, and what helps.

The alarm is ringing. Something felt like rejection.

**New here?** Five quick questions pick the three tools most likely to help you.

Chapter 2 of 9 · Sound like you?

Does this sound like you?

Adults with ADHD described these in research interviews.[Rowney-Smith 2026](https://clearingintheforest.org/?page_id=25#rowney-smith-2026)
[Ginapp 2023](https://clearingintheforest.org/?page_id=25#ginapp-2023)[Beaton 2022](https://clearingintheforest.org/?page_id=25#beaton-2022a)

Chapter 3 of 9 · Three names

One feeling. Three names. Experts disagree.

Tap each name to see its best case and its weak spot.

William Dodson, psychiatrist

### Rejection Sensitive Dysphoria

Says

A sudden, intense pain from real or imagined rejection or failure. A built-in part
of ADHD.[Dodson 2017](https://clearingintheforest.org/?page_id=25#dodson-2017)

Best evidence

Many people with ADHD recognise it at once. 33 of 43 young adults in one interview
study described it.[Ginapp 2023](https://clearingintheforest.org/?page_id=25#ginapp-2023)

Weak spot

No agreed definition, no tested questionnaire, no controlled study. His "98 to 99%"
comes from his own clinic.[Dodson 2023](https://clearingintheforest.org/?page_id=25#dodson-2023)
[Kahn 2026](https://clearingintheforest.org/?page_id=25#kahn-2026) Not in DSM-5-
TR or ICD-11.

Russell Barkley, psychologist

### Emotional dysregulation

Says

ADHD weakens the brakes on every strong feeling. Rejection is one trigger of many.
A new label adds nothing.[Barkley video](https://clearingintheforest.org/?page_id=25#barkley-video)
[Barkley 2015](https://clearingintheforest.org/?page_id=25#barkley-2015)

Best evidence

Severe dysregulation in 55% of adults with ADHD, against 3% without.[Surman 2013](https://clearingintheforest.org/?page_id=25#surman-2013)

Weak spot

He says RSD has no evidence at all, yet studies link ADHD to rejection sensitivity.
Experts still debate whether this is core to ADHD.[Shaw 2014](https://clearingintheforest.org/?page_id=25#shaw-2014)

Geraldine Downey, psychologist

### Rejection sensitivity

Says

Some people expect rejection, spot it fast and react strongly. A measurable trait,
found in many conditions, not only ADHD.[Downey 1996](https://clearingintheforest.org/?page_id=25#downey-1996)

Best evidence

Tested since 1996. Linked to depression and anxiety across 75 studies.[Gao 2017](https://clearingintheforest.org/?page_id=25#gao-2017)
Higher in young people with ADHD symptoms.[Bondü 2015](https://clearingintheforest.org/?page_id=25#bondu-2015)
[Babinski 2019](https://clearingintheforest.org/?page_id=25#babinski-2019)

Weak spot

In adults the ADHD link is mixed. One study of young men found no difference.[Canu 2007](https://clearingintheforest.org/?page_id=25#canu-2007)

Chapter 4 of 9 · The loop

How fear makes itself come true

Downey found a four-step loop.[Downey 1998](https://clearingintheforest.org/?page_id=25#downey-1998)
[Romero-Canyas 2010](https://clearingintheforest.org/?page_id=25#romero-canyas-2010)
Tap a step to see where to break it.

Chapter 5 of 9 · Four brakes

Four brakes on a big feeling

In ADHD the first reaction is faster and the brakes are weaker.[Barkley 2015](https://clearingintheforest.org/?page_id=25#barkley-2015)
[Gross 1998](https://clearingintheforest.org/?page_id=25#gross-1998) Set how hard
it hit, then press each brake.

How hard did it hit? 8 / 10

Press a brake.

Chapter 6 of 9 · The numbers

[Shaw 2014](https://clearingintheforest.org/?page_id=25#shaw-2014)

The biggest numbers come from the loosest methods. The careful study still shows
a large gap.

Chapter 7 of 9 · Treatment

What has actually been tested

### ADHD medicine

 Moderate

Calms emotional swings, about half as well as it helps focus.[Moukhtarian 2017](https://clearingintheforest.org/?page_id=25#moukhtarian-2017)
[Lenzi 2018](https://clearingintheforest.org/?page_id=25#lenzi-2018)

### Guanfacine or clonidine for RSD

 Untested

Dodson reports 1 in 3 respond. No controlled trial exists.[Dodson 2017](https://clearingintheforest.org/?page_id=25#dodson-2017)
[Dodson 2024](https://clearingintheforest.org/?page_id=25#dodson-2024b)

### CBT and DBT skills

 Moderate

Helped ADHD adults in trials, though rejection itself was not measured.[Safren 2010](https://clearingintheforest.org/?page_id=25#safren-2010)
[Halmøy 2022](https://clearingintheforest.org/?page_id=25#halmoy-2022)

### Mindfulness

 Weak to moderate

Small trials show less emotional dysregulation after 8 weeks.[Mitchell 2017](https://clearingintheforest.org/?page_id=25#mitchell-2017)
[Ghonchehpour 2023](https://clearingintheforest.org/?page_id=25#ghonchehpour-2023)

**Never start, stop or change a medicine on your own.** Take this to your prescriber.

Chapter 8 of 9 · The verdict

So who is right?

### Both agree

The pain is real, common in ADHD, and skills plus ADHD treatment help.

### RSD goes too far

A separate condition, nearly universal in ADHD, treated with untested medicines.

### Barkley goes too far

"No evidence at all." Several studies do link ADHD and rejection sensitivity.[Babinski 2019](https://clearingintheforest.org/?page_id=25#babinski-2019)
[Müller 2024](https://clearingintheforest.org/?page_id=25#muller-2024)

Chapter 9 of 9 · Get help

When it is more than a bad moment

See your GP or ADHD team if low mood lasts more than two weeks, if the hurt is wrecking
work or relationships, or if you feel hopeless. Mood that stays low or high whatever
happens may be a separate condition.[Shaw 2014](https://clearingintheforest.org/?page_id=25#shaw-2014)
